• Payor Name
• Social Security Number
• Payor Address (address 1, address 2, City, State, ZIP, Country)
• Bank name, account number, bank routing number  
• Valid Non-custodial Parent Case Number *
• E-Mail Address

*If the Non-custodial Parent has multiple case numbers, only one is needed to register
